Product selections form

Welcome to our on-line Product Selection Form. We hope you’ll find it to be an enjoyable way to view and record the products you want for your installation. A copy of your choices will be emailed to both you and Millwerx, so we can discuss changes and customizations from there.
A few quick instructions before you start:

Use a computer or laptop to use the form.

The pages scroll down. Click “Next” to go to each new screen.

Important: To exit a supplier website and return to your form page, click the orange Millwerx form icon along the top of your browser - DO NOT exit by clicking the left back arrow on the top left of your screen.

The form does not have a save function, so we recommend you complete it in one sitting.

If you need to make a change or have forgotten something on the form, simply give us a call within 48 hours. No stress.
